Telehealth counseling is an alternative to traditional counseling services for those who wish to seek services from the convenience of their own home, who may have a schedule that makes it difficult to come into the office regularly, or who may need time to talk in between visits to the office. 

Getting started is as easy as 1-2-3!

1. You may utilize my free 15 minute consultation to answer any questions you may have for me. You also have the option to give me a call at (941) 677-8195, or email me at Stephanie@CenterForCMS.com to schedule your first telehealth session.

2. Then complete and fax me the New Client Intake and Informed Consent on the Forms Page    to 941-907-8896. They can also be scanned to Stephanie@CenterForCMS.com.   
 
3. Make a payment. You can use the PayPal link provided below. Initial telehealth and followup sessions must be pre-paid according to the fee schedule.

Telehealth counseling can begin only after I receive the required forms and payment.
